Enemies are not anomalies

Anomalies try to walk in looking almost normal. Enemies are already hostile once they appear. You cannot reject a Stalker or Bed Monster at the shutters the way you reject a three-eyed patient. Treat this page as combat and spawn rules: where it shows up, how hard it hits, and what actually works.

A few names overlap other pages on purpose. Head Banger and Shapeshifter also show up as special threats on the anomalies side. Bed Monster and Tendril also show up as timed emergencies. Here the focus stays on the monster itself: damage, counter, and how not to make things worse.

What to carry before they show up

Sanity is the real health bar. Most enemies are designed to melt it while you are still trying to treat patients.

  • Keep coffee ready for Head Banger and for your own sanity bar.
  • Carry a taser or gun for Hiders, Ghost, and Shapeshifter fights.
  • Keep a fire extinguisher for Ghost reveals and fire problems that can chain into worse chaos.
  • Bring maple syrup (or a transplant/organ backup) before you walk the bed red zones in rooms 1-5 and 7.
  • Eye drops help if Mass of Eyes is already on the ceiling and you cannot finish the room without looking.

How to clear each threat family

Roaming threats punish bad camera habits and empty hands. Angle low around corners for Stalker. Listen for Hider breathing on walls, then stun or kite them into a Bed Monster if you are empty-handed. After a patient death, hunt the Ghost with extinguisher spray first so you can actually see what you are shooting. Shapeshifter is the late form after a bad admit: when the huge mouth drops, stop treating and start fighting.

Room threats stay tied to beds, ceilings, and Room 8. Never stand in a Bed Monster red line without bait. For Mass of Eyes, believe the “don’t look up” warning and finish the patient with your head down. Tendril is surgery only: the mini-game saves the patient, and a taser shot kills them.

Reception and camera threats are desk jobs, not hallway chases. Give Head Banger coffee instead of hard-rejecting it for a -20 sanity scare. On CCTV, leave broken feeds or repair them before Camera Figure finishes the zoom-in jumpscare.
