Each row keeps both source spawn locations, the required weapon or vehicle method, and the target objective. The first in-game pickup remains available at both listed locations if a previous attempt fails.

How many locations can each Rampage use?

Each challenge has two possible spawn locations in the source table, and the pickup remains available at both locations until completed.

What happens after a failed attempt?

The source says the Rampage can be attempted again, so a failed run does not permanently remove the challenge.
