A successful escape is enough for some achievements. Others care about the gear you brought, what you carried out or whether you fired at all. Pick your goal before the raid, because those conditions can pull you in different directions.

Plan around Active Matter's raid conditions
Read the action as carefully as the target. An achievement that asks you to extract needs a successful escape; visiting a raid or using something inside it is a different requirement. A location name matters too. Cargo Port and Headquarters objectives belong to those raids, rather than any raid with similar loot.
Watch for conditions that apply to a single run. The Floor Is Lava requires its trap activations in one raid, while Active pockets explicitly adds up extracted Active Matter across raids. Those are different kinds of progress, so don't assume every target resets or carries over the same way.
Shelter goals and Hired Ops use different activities
The Replicator's goals separate item production, recipe variety, blueprint unlocks and chronogene fusion. Unlocking Body Sleeves and equipping their chronogene slots are separate tasks too. If you've done plenty of crafting but an achievement is still locked, make sure you've done the particular action it asks for.
Hired Ops has its own beacon-capture and Combat Master goals. Investigation Terminal cases and Priority Raid objectives also have distinct achievements. For the location objectives, Maps and Raids covers Cargo Port and Headquarters.
Hidden Steam achievements still count
Steam masks the names and conditions of hidden achievements until you unlock them or reveal their spoilers. They still belong to the same achievement set. A job no one else could do asks for every other achievement, so the hidden goals are part of that requirement too.
