Enter the hospital through the main entrance. Take a ladder from the inventory. A broken wall is located above the desk. Place the ladder beneath it. Climb the ladder and enter the broken wall to access the secret room. Go to the emergency room roof next to the hospital. Climb onto the roof (a ladder may be required). Walk to the opposite side of the roof. Enter through the broken window. A hidden room is discovered.

Look underneath the desk to find a blue button. Press the blue button. To exit, look at the small wooden stool and press the blue button to teleport out.

Agency Room A hidden Agency Room exists beneath the Power Plant. Accessible via the Reactor Room using manholes and corridors. Requires a Blue Key Card located in Cell #2 of the Police Station. Contains Agency-related equipment and computers. Meltdown Event and Green Crystal A meltdown can be triggered using C4, waste room levers, and cooling valve shutdowns. Sirens and warning lights activate during the meltdown. Interacting with the reactor core grants a Green Crystal, which currently has no known use. Agency symbols suggest a larger hidden storyline.
