The Outlast Trials is a horror game where you and your teammates are test subjects trying to survive Murkoff's experiments. Prescriptions are upgrades that make your character faster, tougher, or better at escaping danger. They're split into three tiers, and you buy them from the pharmacy inside the game.

What Are Prescriptions in The Outlast Trials?
Prescriptions are upgrades that stay with your character once you buy them. They work more like permanent skills than items you find during a trial. Some apply on their own, such as Speed Boost, which makes you run faster all the time. Others you trigger yourself, such as Slide, which you use by crouching while you run. Nothing runs on a timer, so there are no cooldowns to plan around.
How to Unlock and Buy Prescriptions
Nurse Emily Barlow sells Prescriptions at the pharmacy in the Sleep Room, but she won't deal with you until you reach Therapy Level 3. You get there by completing trials, and the better your grade, the more XP you earn. Purchases use Vouchers, which you mostly earn by leveling up your Therapy Level, and the same Vouchers also pay for Rigs and Amps. Vouchers come in slowly, so it pays to think before you spend them.
How the Three Prescription Tiers Work
Prescriptions are split into three tiers that unlock in order. You have to buy every Tier 1 Prescription before Tier 2 opens, and the same rule applies to Tier 3, so you can't skip ahead to the ones you want most.
Each tier also has a loose theme. Tier 1 leans toward movement and escaping, Tier 2 toward stamina and speed, and Tier 3 toward health, reviving, and getting out of traps and ambushes. Knowing that helps you match your early buys to how you like to play.
Best Prescriptions to Get First
A handful of picks come up again and again, so they're a safe place to start. In Tier 2, Speed Boost, Endurance, and Athlete help most, since running out of stamina is one of the quickest ways to get caught. Battery Extension and Expanded Inventory are popular too, because they save you from hunting for batteries and from running out of room for healing items. Gymnast usually comes last, since you spend more time running and bashing than jumping and climbing.
In Tier 1, Instant Use and Slide are common early picks, and Regeneration is a good choice for newer players because it heals you over time. In Tier 3, Stamina Recovery, Revitalize, and Resistance are the ones most players grab early. Revitalize fully restores your health when you respawn in solo play or get revived with a syringe, so it can prevent a death. Resistance helps you break free from traps and enemy grabs faster, which matters because those can't always be avoided.
Prescriptions for Solo and Group Play
Not every Prescription helps everyone equally. Hard Kick and Stay of Execution only matter when you have teammates, since they save your stamina when you help someone out of a grab or an execution. Medic is a group pick too, because it makes reviving teammates faster. If you play alone, Heavy Training and Doors Basher are more useful, since you'll be the one carrying heavy objects and getting through locked doors.
FAQ
Do I have to pick my Prescriptions before every trial?
No. Once you buy a Prescription, it stays active for your character, so there's nothing to choose before you head in. You can check what you own in the Loadout menu at the central terminal.
What's the difference between Prescriptions and Rigs?
Prescriptions are passive upgrades that you buy once and keep. Rigs are gear you choose yourself. You can own several, but only one can be equipped for each trial, so you pick it in your loadout before you go in.
Which Prescription helps if a Pouncer grabs you?
Self Defense Technique. It lets you use a bottle or brick from your inventory to break free of a Pouncer's grip. Resistance also helps, since it makes you break free from grabbing enemies faster.
